What Virginia Inspectors Cite Most Often in Richmond-Area Assisted Living

We tallied every standard VDSS cited at licensed assisted living facilities in our twelve localities since January 2025. Service plans, staff records and medication handling top the list.

Quick answer

Every licensed assisted living facility in Virginia has a public record on the Virginia Department of Social Services (VDSS) search, and each posted inspection lists the standards an inspector found out of compliance. We pulled those records in October 2026 for all 128 licensed facilities in Richmond City, Henrico, Chesterfield, Hanover, Goochland, Powhatan, New Kent, Petersburg, Colonial Heights, Hopewell, Prince George and Dinwiddie, and counted every standard cited at inspections dated January 1, 2025 or later.

TL;DR

Across 529 posted VDSS inspections since January 2025 at Richmond-region assisted living facilities, 211 found at least one violation and 533 standards were cited in all. The most-cited rules cover individualized service plans, staff records, medication administration, resident physicals and personal care. Use the list to shape your tour questions.

What did we count, and where does the data come from?

Every licensed assisted living facility in Virginia has a public record on the Virginia Department of Social Services (VDSS) search, and each posted inspection lists the standards an inspector found out of compliance. We pulled those records in October 2026 for all 128 licensed facilities in Richmond City, Henrico, Chesterfield, Hanover, Goochland, Powhatan, New Kent, Petersburg, Colonial Heights, Hopewell, Prince George and Dinwiddie, and counted every standard cited at inspections dated January 1, 2025 or later.

That gave us 529 inspections. Of those, 211 recorded at least one violation, 222 were complaint-related, and inspectors cited 533 standards in total. A single inspection can cite several standards, and many cite none. These are the state's findings, not ours; we only grouped them by the section of the regulation, 22VAC40-73, that each citation falls under.

Which assisted living rules are cited most often around Richmond?

Five sections of the standards account for a large share of all citations in the region, and none of them is about décor or amenities. They are about paperwork that protects residents and the hands-on work of care.

RankStandard (22VAC40-73-)What it coversCitations since Jan 2025
1450Individualized service plans38

7950 / 870 / 70Emergency plans; building maintenance; incident reports19 each

Counts are by regulation section across all localities. A facility with many inspections, often because complaints prompted extra visits, will contribute more citations than a facility inspected only once a year, so the totals describe the region, not any single building.

Why do individualized service plans get cited so often?

An individualized service plan is the written plan, built from the Uniform Assessment Instrument, that says what help a resident gets and how often. Virginia requires a preliminary plan on or within seven days before admission and a comprehensive plan after, updated as needs change. Citations under section 450 typically mean a plan was missing an element, was not updated after a change, or did not match what the resident actually needed.

For a family, the practical takeaway is simple: ask to see your parent's service plan after move-in and after any hospital stay, fall or change in medications. Check that it reflects the doctor's orders and anything the resident has said they want, such as a do-not-resuscitate order or a preference about bathing times.

What do staff-record and medication citations tell a family?

Section 250 citations are about staff files: health requirements such as tuberculosis screening, and documentation that the facility verified what it is supposed to verify before someone works with residents. A pattern of these suggests a facility struggling with hiring paperwork, which often goes along with turnover.

Medication administration under section 680 is where mistakes reach residents directly. Citations can involve missed doses, medications given without a current order, or records that do not match what was given. On a tour, ask who passes medications on each shift, what training they have, how the facility checks the medication record against pharmacy deliveries, and how it tells families about a medication error.

How should I use these findings when touring a Richmond facility?

Use them to turn a sales tour into a real conversation. Open the facility's VDSS record before you go, note any citations in the last two years, and ask the administrator what changed afterward. A good administrator will answer specifically; the plan of correction is not posted online, but the facility can describe it.

Then ask about the areas that show up most often across the region even if this facility was not cited: how service plans are reviewed, how new staff are screened, how medications are double-checked, and how incidents are reported to families. You can browse every facility's last inspection date and citation count in our sortable directory.

Remember that no citations is not the same as excellent care, and a single citation is not a verdict. Look for patterns over time, repeat citations of the same rule, and how quickly problems were fixed.

Do complaint inspections mean a facility is worse?

Not necessarily. Complaint-related inspections made up about four in ten posted inspections in our count, and many found no violation. Larger facilities serve more residents and families, so they tend to generate more complaints in absolute terms.

What matters is what the complaint inspections found. Open each one on the VDSS search; the summary lists the areas reviewed and any standards cited, and a complaint that was substantiated on supervision or medications deserves a direct question.
